K-12- How can our former students order replacement diplomas?

Students can complete this form, and a member of your Regional Diploma Team will walk them through ordering a new high school diploma.

They will be asked to submit:

1. HJ Replacement Diploma Form
2. Official, sealed transcripts
3. Check or money order for the production cost and shipping.

*Please include the high school’s name and location. Florida schools should also include the county in which the school is located.
